    • Proof of Travel:
    • copy of roundtrip ticket, flight itinerary, or letter from tour agency/operator
    • Letter of Intent, if applying for tourist visa

    If Traveling on Business:
    • Business Letter Required: Business Letter Format

    • Proof of legal status for Non US Citizens:
    Proof of legal status (for non US)
      - Valid US Visa (ie: H1B, F1, J1, E)
      - Advance Parole Document
      - Valid DS2019 (formerly IAP66)
      - Valid I-94
      - Valid I-20 (signed within 3 months by designated school official)
      - Photocopy of Green Card
