Both visa application and supplemental information forms are fillable pdf forms. Forms must be TYPED using your computer and printed. Please be advised handwritten visa forms are no longer accepted at the embassy.

B) Purpose of the visit, date of arrival and departure, places to be visited, relationship between the applicant and inviter, and who will bear the cost of the applicant's accommodations in China
C) Inviter's info: name of the unit or individual, phone number, address and if applicable, seal and signature of the legal representative.
