    Please gather the following documents

    1. COMPLETED ONLINE DS-5504 FORM
    2. CURRENT VALID PASSPORT
    3. TWO IDENTICAL PHOTOGRAPHS
    4. PAYMENT: $60 (to satisfy the US passport agency)
    5. SUPPORTING DOCUMENT IN THE ORIGINAL:
    a. Marriage Certificate
    b. Divorce Decree
    c. Birth Certificate
    d. Adoption Decree
    e. Court Order of a Name Change
     6. PROOF OF TRAVEL:
    A copy of the airline ticket or travel itinerary demonstrating your impending departure within ten (10) business days (if no visa is required) or within a month if a visa is required of the date of application.
    A business letter can also serve as proof of travel.
