- If you are behind in child support payments, you may not be able to get a passport. See Child Support .
- Minors who have been issued a five year validity passport must always apply for a first time passport and submit the original birth certificate.
- Form DS64 form: A statement regarding Lost or Stolen Passport must then accompany this application if you cannot locate your original passport.
