Please gather the following documents
1. COMPLETED ONLINE DS-82 FORM
2. CURRENT VALID PASSPORT
3. TWO IDENTICAL PHOTOGRAPHS
4. PAYMENT: $170 (effective July 13, 2010 to satisfy the US passport agency)
5. PROOF OF TRAVEL:
A copy of the airline ticket or travel itinerary demonstrating your impending departure within ten (10) business days (if no visa is required) or within a month if a visa is required of the date of application.6. A LETTER OF AUTHORIZATION
A business letter can also serve as proof of travel.
